included on the LDA-I newsletter and website, and be announced at the annual LDA-I

A current IEP, psychological report, or similar document with proof of a
learning disability is required to accompany this application. One of these
documents must be included to be considered for the scholarship.
The Scholarship Committee requests an essay from scholarship applicants. This
information includes:
1. Comments that allow the Committee to know more about you.

2. Other information about your college/career goals.
Your statement will be read by the scholarship committee members and will be
treated with confidentiality. This section must be completed by the applicant to
be considered for the scholarship.

extra-curricular and community achievements, most significant accomplishments,
volunteer and paid jobs, and methods of financing your education. You may also
wish to include other information that you feel the committee needs to know.
